Prescribing scope is the biggest single lever
The difference between an ANP earning £48 an hour and an ANP earning £75 an hour is almost never clinical ability. It is whether they hold independent prescriber status, and whether their documented scope of practice covers the specific pathway the provider is staffing.
Not what you feel competent to do. What is written down, supervised, evidenced by CPD and auditable. Providers cannot pay a premium for competence they cannot evidence to a commissioner or a regulator.
The highest-paying pathways for ANP prescribers right now are ADHD titration, where controlled drugs scope commands a premium, weight management and GLP-1 review, and menopause and HRT where specialist accreditation is increasingly expected.
Where independent ANP demand is concentrated
Four pockets absorb most independent ANP hiring in the UK. Remote primary care, where you hold a general caseload with safety-netting discipline. Weight management, structured and protocol-led with high repeat volume. ADHD assessment and titration, the fastest-growing and the one requiring the tightest governance. And long-term condition review inside employer and PMI-backed services.
Each pays differently and each demands a different scope. An ANP who maintains a broad, well-evidenced scope can move between all four and effectively price their own sessions. An ANP whose scope was set by one previous employer is limited to whatever that employer happened to cover.
Autonomy is not the same as being unsupported
Advanced practice in the independent sector genuinely offers more clinical autonomy than most NHS Band 7 posts. It should not offer less support. Before accepting any independent ANP role, establish who the responsible clinician for the pathway is, what the supervision line looks like, how your decisions are audited and sampled, and what the escalation route is when you decline to treat.
You carry personal NMC accountability regardless of what a provider protocol says. A role with high autonomy and no supervision structure is not a senior role, it is an exposed one.
Salaried against sessional
Sessional ANP work pays a higher headline rate but carries no sick pay, no annual leave, no pension contribution and no notice protection. A rough working comparison is that a sessional rate needs to be around 25 to 30 percent above the salaried hourly equivalent before it is genuinely better paid.
Sessional work across multiple providers with genuine clinical autonomy is usually engaged outside IR35. A fixed rota with mandated hours and no substitution right is less clear cut and worth specific advice.
